With the normal brain MRI DIS is not fulfilled. The spinal cord lesion of more than three vertebral segments points to NMO (also called Devic's disease) as for NMO MRI of the spinal cord demonstrates lesions typically extending contiguously over three or more vertebral segments on sagittal T2-weighted images. Lesions are usually located in the cord center and enhance with Gadolinium, sometimes for several months.

In the early phase of NMO brain MRI is normal.

NMO should be separated from typical MS because of different underlying pathophysiology, clinical course, prognosis and treatment.

The diagnosis of MS can not be made.
